The importance of complete and accurate clinical records is well recognised.
Any optometrist who has undertaken locum work will be aware of the diversity of record cards in use. With domiciliary work in particular, neat and legible records are difficult to achieve. These printable labels can be attached to any record card to quickly record the results of ophthalmoscopy, using a highlighting pen. The format is quickly recognized
by the next optometrist who looks at the patient record.
Small changes in, for example, optic disc appearance, become more evident between successive examinations, and grading of clinical conditions can be made.
As has been stated elsewhere many times – if it’s not noted down, it wasn’t done! |
